Description
The Multi-Cultural Operations area of the Human Services and Public Health department is looking for a Case Management Assistant (CMA) who is fluent in another language, in addition to English to join our team. This position will assist Hennepin County residents gain access to services to supports their needs, allowing them to live healthy, safe, stable, and independent lives.
Location and hours:
This position is hybrid and will be performed both on-site at the South Minneapolis Human Services Center, 2215 East Lake Street Minneapolis, MN 55407, in the community having direct contact with residents and remote as job duties require. Work hours will be Monday through Friday, with typical hours of 8 a.m. to 5 p.m. with flexible scheduling including some occasional weekend/evening shifts. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.

- Seek out, interview, and assist residents in need of agency services in their homes (if necessary), to explain supportive services and resources available to help them meet their needs.
- Seek out, participate and attend community events to build relationships with county residents and community organizations.
- Serve as a cultural liaison between county staff and members of the community.
- Interact with schools, medical personnel, and other agencies to facilitate communication between them and our mutual clients.
- Inform county residents about available resources and assist them in completing applications and making appointments.
- Work closely with Community Engagement Coordinator in pushing engagement work across our area.
- Network, collaborate, and partner with internal and external partners and Community Engagement Coordinator to advance community engagement work across our area.
- Ability to communicate fluently in another language, in addition to English, both verbally and in writing.

One of the following:
- Two or more years of responsible, paid social service experience providing direct client service.
- A two-year human services certificate.
- Two or more years of college coursework in the behavioral sciences field.
- Knowledge of community resources and social services available to serve clients.

Experience:
- With detailed interview and note taking techniques.
- Using databases and computer programs such as Social Services Information System (SSIS), Electronic Case File (ECF), and Medicaid Management Information System (MMIS) to perform data entry where accuracy is essential.
- Working with multiple providers, staff, and collaborative partners to improve the care of diverse and underserved people with a variety of cultural and language barriers.
- Using Microsoft Office Suite (Excel, Outlook, PowerPoint, Teams, and Word).

Ability to:
- Work effectively and communicate with individuals from a variety of cultural and lifestyle backgrounds in a respectful and non-judgmental manner.
- Make sound, independent judgment.
- Work both independently and collaboratively as an effective team member.
