What are the responsibilities and job description for the Community Health Specialist (Mobility Assignment internal Hennepin County applicants only) position at Hennepin County?
Hennepin Health is seeking two Community Health Specialists to join the Special Needs BasicCare (SNBC) team to provide care coordination for members who are certified disabled focusing on the needs of members with mild-moderate complexity. These positions will engage directly with members to offer care coordination and support including establishing primary care, addressing systemic barriers and facilitating other supportive resources as applicable.
These Community Health Specialists will work within an interdisciplinary team of nurses, other community health specialists, and a social worker to sensitively address the social determinants of health of SNBC members. These roles will focus on a vulnerable population and work to address their needs in a trauma informed and member-centric way. Individuals filling this role must be flexible, innovative, and understanding of the challenges and unique health conditions experienced within the SNBC population.
You must be a current regular or probationary employee of Hennepin County to be eligible for these positions. These positions are job mobility assignments, with participation for up to 12 months, with the possibility to extend up to 18 months. A mobility assignment is a voluntary job change to a different department that is filled by a regular or probationary employee to carry out the duties of the same or a different job class. Hennepin County mobility assignments allow employees to develop a particular expertise and prepare for promotional opportunities as well as facilitate, support, and accelerate knowledge transfer across departments. Job mobility assignments are temporary; employees typically return to their original department at the conclusion of their mobility assignment.

In these positions, you will:
- Facilitate annual completion of the health risk assessment, reassessments and support plans; provide appropriate follow up to members. Conduct this work in the field, community settings, and telephonically to accommodate member needs.
- Manage caseload of SNBC members with varied disabilities and needs; providing responsive follow up and management of individualized care plan; conduct quarterly check-ins with members.
- Work within MNChoices and Health Edge, Guiding Care (care management system) to process assessments, care plans and other relevant care coordination forms; prepare and maintain records including operational, regulatory, and utilization reports adhering to all contractual requirements specified by the Minnesota Department of Human Services (DHS).
- Participate in team, member, and all staff meetings; establish and maintain effective relationships with other case management staff to effectively coordinate member care.
- One of the following:
- Bachelor's degree or higher in public health, nursing, behavioral/social science, or an approved related field.
- Four years or more of related professional experience providing direct service to assess and support client needs, organizing and reporting on programmatic data, health education, counseling, outreach, and referral services with the community.
Experience:
- Addressing racial equity, gender equity, LGBTQIA equity, neurodiversity equity, differing ability equity.
- Managing a caseload within a team providing case management or care coordination services in a clinical or community-based setting.
- Navigating social services and housing options available to residents of Hennepin County.
- Working with Minnesota Health Care Programs.
- Using electronic health records, MnChoices, Microsoft Office Suite particularly Teams, Excel, and other related computer applications.
Knowledge of:
- Principles and practices of care coordination; service referral within a county system.
- Information systems relevant to care management, coordination, and reporting.
- The unique challenges experienced by those impacted by mental health and chemical health problems as well as the impact of complex trauma history.
- Trauma informed and harm reduction approaches to care.
Ability to:
- Work independently in various settings supporting individuals who are experiencing marginalization and distress.
- Act in an advocacy role on behalf of members.
- Ability to function well in unpredictable and challenging situations calling for sensitivity, flexibility, resourcefulness, and independent judgement.
