NorthPoint Health & Wellness Center is seeking a Nurse Practitioner to provide a full spectrum of health care services in their community clinic. This position will also provide aid for substance use disorder (SUD) same-day services, co-occurring mental illness and SUD wound care. Our practice is a diverse, urban, community health center in North Minneapolis whose mission is “Partnering to create a healthier community.”

Location and hours:
This position will be performed on-site at both the NorthPoint Health & Wellness Center at 2220 Plymouth Avenue North, Minneapolis, and at the 800 West Broadway Clinic at 800 West Broadway Avenue, Minneapolis, MN 55411. Work hours will be Monday through Friday, with typical hours of 8 a.m. to 5 p.m. New Employee Orientation (NEO) for all new benefit-earning employees is in-person in downtown Minneapolis. In this position, you will:
- Perform initial and on-going assessment of patients' medical, physical, and psychosocial status.
- Obtain and evaluate health and medical history, identify medical and health risks, and triage those who require physician consultation.
- Conduct preventative screening procedures and perform physical examinations based on age and history, complete accurate and timely documentation of observations, interventions, plan of care, referrals and follow up, and develop client education plans.
- Provide same-day services for diagnosis and referral of myriad conditions with specific attention being paid to SUD.
- Make appropriate referrals to other health professionals and community agencies based on assessment of co-occurring conditions, with particular attention being paid to medication assisted therapy.
- Participate in chemical health care coordination with behavioral health, human services departments, and external treatment providers.
- Aid in the development and execution of integrated whole person SUD Programming including medication assisted treatment, wound care and addressing social determinants of health through collaboration with other internal departments.
- Order, conduct, and interpret diagnostic tests, order appropriate therapeutic treatments, and prescribe, administer, and dispense therapeutic devices and legend drugs.
Need to have:
One of the following:
- Possession of a valid license as a Registered Nurse issued by the State of Minnesota and a National Certification as a Nurse Practitioner (CNP).
- Possession of a valid license as an Advanced Practice Registered Nurse issued by the State of Minnesota.

- Experience:
- As a Nurse Practitioner.
- With SUD same-day services, co-occurring mental illness, and SUD wound care.
- Providing chemical health care coordination with behavioral health, human services departments, and external treatment providers.
- Working with populations diverse in race, ethnicity, age, gender, identity, ability, and sexual orientation.
- In clinical quality improvement work, implementing clinical change cycles, monitoring data for progress, and/or resetting clinical priorities.
- Utilizing Epic or another similar electronic health records system.
- Ability to:
- Demonstrate both verbal and written communication skills.
- Demonstrate problem-solving skills and adapt to an ever-changing clinic practice environment.
- Respect, appreciate, and promote teamwork among diverse co-workers and patients regarding race, gender, disability, values, and lifestyle perspectives or interests.
