Integrated Case Manager

Job Specialty :

Job Duration : 13 weeks

Shift :

8 : 30 AM - 5 PM, rotating schedule with weekend availability required

Guaranteed Hours : 40 hours per week

Experience :

3-5 years of case management experience in an acute inpatient setting

License :

Registered Nurse (RN) with a valid, unrestricted State license or Licensed Social Worker (LMSW) with a valid, unrestricted State license

Certifications :

Basic Life Support (BLS) certification from the American Heart Association

Must-Have :

Experience with Epic EMR
Strong computer skills
Excellent verbal communication and written documentation skills

Job Description :

Conduct comprehensive assessments of patients and their family / caregivers to determine biomedical, psychological, social, and functional needs.

Develop personalized, patient-centered care plans to optimize the patient's care experience.

Engage patients and families as part of the care team through advocacy, communication, health education, resource identification, and service facilitation.

Utilize professional judgment, critical thinking, motivational interviewing, and self-management techniques to help patients overcome barriers to goal achievement.

Provide counseling and interventions related to treatment decisions and end-of-life issues, including Advanced Care Planning.

Ensure seamless and safe transitions for patients between care settings.

Advocate for the appropriate delivery of services within the patient's health plan benefit structure.

Collaborate with members of the patient's treatment / care team to manage patients with complex medical and social needs and facilitate collaborative care conferences.

Continually evaluate the patient's response to the care / treatment plan and make modifications when necessary.

Plan and participate in process improvement activities to reduce risk, including data collection, analysis, and follow-up interventions.

Facilitate interventions in cases involving child abuse and neglect, domestic violence, elder abuse, institutional abuse, and sexual assault.

Support department-based goals contributing to the success of the organization.

Perform other duties as assigned.

Candidates for this position should possess excellent customer service and interpersonal skills, strong problem-solving, analytical, and decision-making skills, and a strong sense of compassion and advocacy for patients and their families.
