Overview
We are seeking a detail-oriented and customer-focused Admissions Coordinator to join our admissions team. The Admissions Coordinator will play a vital role in supporting the admissions process, from initial inquiry to admitting patients to the facility. This individual will serve as a primary point of contact for patients, providing information, guidance, and assistance throughout the admissions process.
Responsibilities
- Inquiry Management: Respond promptly and courteously to inquiries from prospective patients, and case workers via phone, email, and in-person visits.
- Application Review Support: Support admissions counselors in reviewing applications by organizing and maintaining applications, preparing materials for review, and facilitating communication between Case Managers/SNF Admissions and Heritage Admissions staff.
- Caseworker Outreach: Conduct outreach to prospective patients through various channels, including phone calls, emails to case managers, insurance specialists, hospitals, nursing homes, or other medical centers.
- Responds to emails.
- Complete assigned authorization, audits, and other data validation,
- Provide accurate information about the admissions process, requirements, and program.
- Review referrals for completeness and accuracy, following up with CMs/SNF Admissions as needed to obtain missing information.
- Enter application data into the admissions database and maintain accurate records.
- Maintain communication about admissions, hospitalizations, and pending discharges.
- Build rapport with case managers and serve as a resource throughout the admissions process.
- Assist with special projects and initiatives as assigned by the Admissions Director or designee.
Experience
- Minimum of 1-2 years of experience in admissions, customer service, or healthcare-related field.
- Excellent written communication and interpersonal skills, with the ability to interact professionally with diverse populations.
- Strong organizational skills and attention to detail, with the ability to manage multiple tasks simultaneously.
- Proficiency in Microsoft Office and Excel preferred but not required.
- Ability to work independently and collaboratively in a fast-paced, deadline-driven environment.
- Commitment to providing exceptional customer service and fostering a positive experience for prospective patients, Heritage staff, and our referral sources.
- Basic knowledge of Healthcare Insurance and Prior Authorizations is preferred but not required.
