Heritage Group Homes, Inc. is seeking a highly skilled and experienced Bilingual Lead Care Manager to join our Enhanced Care Management (ECM) program. The Lead Care Manager will be responsible for overseeing the coordination and delivery of care management services, ensuring that clients receive comprehensive, high-quality care. The ideal candidate must have experience with Electronic Health Records (EHR) systems and a strong background in care management within a non-profit setting.
Key Responsibilities:
- Care Coordination: Lead and oversee the care management team in coordinating and delivering comprehensive care management services to clients.
- Assessment and Planning: Conduct thorough assessments of clients' needs and develop individualized care plans that address clinical, non-clinical, and social determinants of health (SDOH).
- Team Leadership: Provide supervision, training, and support to care managers and other staff members to ensure the effective implementation of care plans.
- EHR Management: Utilize Electronic Health Records (EHR) systems to document client interactions, track progress, and ensure accurate and timely record-keeping.
- Client Advocacy: Advocate for clients' needs and work collaboratively with healthcare providers, social service agencies, and community resources to facilitate access to necessary services and support.
- Monitoring and Evaluation: Monitor the effectiveness of care plans and make adjustments as needed to ensure optimal outcomes for clients.
