What are the responsibilities and job description for the Home Health Clinical Supervisor position at Heritage Home Health and Hospice?
Heritage Home Health and Hospice is currently seeking a compassionate and dedicated Licensed or Registered Nurse Clinical Supervisor to join our awesome Home Health team in Texarkana Tx. This position requires strong clinical skills and excellent communication abilities. This is an office position.
- Supervises and provides direction and leadership to clinical nursing and aide personnel to ensure quality and continuity of services.
- Coordinates services. Informs the physician and other personnel of changes in the patient's/client’s condition and needs. Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care. Integrates orders from all relevant physicians involved into the plan of care and ensures the orders are approved by the responsible physician.
- Monitors systems and identifies problem areas to the Director of Clinical Services or the Director of Operations.
Pay is competitive and is based on experience.
