Heritage Home Health Services is currently looking for a full-time/part-time Physical Therapist to join our team! The area of coverage will be determined upon location of candidate.. This is a pay per visit position.

Qualifications:
- Ability to perform therapy evaluations, routine visits, and OASIS documentation
- Work with other disciplines to give the best care needed for each patient.
- Help decrease the need for re-hospitalization by being actively involved in the patient's plan of care to assess any changes that may be needed.
- When appropriate teach and counsel family members about care and processes for disease/injuries.
- Attend orientation, meetings, and CEUs
