Job Title: Director of Nursing, Assisted Living and Memory Care
Position Summary:
The Director of Nursing (DON) for Heritage of Union City Assisted Living and Memory Care is responsible for overseeing the clinical and nursing care operations within the community. This role involves leading and managing the clinical team, ensuring high standards of resident care, compliance with regulatory requirements, and implementing best practices in memory care and assisted living settings. The DON works closely with the Executive Director and other leadership to foster a compassionate, person-centered environment for residents.
Key Responsibilities:
- Resident Care and Clinical Oversight:
- Oversee and direct all aspects of nursing care provided to residents in both Assisted Living and Memory Care units.
- Ensure a high level of care by assessing residents’ healthcare needs, creating personalized care plans, and regularly monitoring their health status.
- Coordinate with physicians, therapists, and other healthcare providers to ensure comprehensive and continuous care.
- Staff Management and Development:
- Recruit, train, and mentor the nursing staff, including RNs, LPNs, QMAs, and CNAs, ensuring a competent and compassionate team.
- Conduct regular performance evaluations, provide feedback, and foster a positive work environment that encourages professional growth.
- Ensure staff adherence to all policies, procedures, and best practices related to resident care and facility operations.
- Regulatory Compliance:
- Ensure all clinical practices comply with local, state, and federal regulations, including infection control, medication management, and safety protocols.
- Maintain accurate and comprehensive documentation of resident care and clinical records.
- Prepare for and participate in regulatory inspections and audits, addressing any areas of improvement as needed.
- Quality Improvement:
- Implement quality assurance and improvement initiatives to enhance the standard of care and resident satisfaction.
- Monitor and evaluate the effectiveness of nursing practices and make adjustments to improve outcomes and safety.
- Address resident and family concerns promptly and effectively to ensure a high level of satisfaction.
- Collaboration and Communication:
- Collaborate with other department heads and the Executive Director to support community goals and enhance resident quality of life.
- Communicate regularly with residents’ families regarding their loved ones' health status, care plans, and any significant changes.
- Participate in interdisciplinary meetings, providing clinical insight and support for community initiatives.
Heritage tracks Integrated Health Care Coordination for Medicaid Residents. Allowable activities include:
- Development and oversight of a healthcare support plan that includes coordination of medical care and proactive care management of both chronic diseases and complex conditions such as recurring falls, depression and dementia. (Note: Skilled nursing services are provided within the scope of the Indiana State Nurse Practice Act.) * Physician collaboration * Medication review * Transitional care from hospital or nursing facility to home/assisted living * Advance care planning
The following activities are not allowed under IHCC: * Skilled nursing services that are available under the Indiana Medicaid State Plan * Any other service otherwise provided by the waiver
Qualifications:
- Education: LPN or RN required.
- Licensure: Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of employment.
- Experience: Minimum of 2-5 years of nursing leadership experience, preferably in assisted living, memory care, or a similar setting.
- Skills: Strong leadership, clinical expertise, organizational, and interpersonal skills; familiarity with memory care best practices; proficiency in healthcare technology and EMR systems.
