Physical Therapy Assistant License in MN (Required)

Primary Responsibilities
Provide physical therapy to home care patients.
Responsible for timely and accurate communication and documentation relative to physical therapy services.
Promote physical therapy services in homecare
Specific Job Duties - includes, but not limited to :
Treat patients suffering from physical disability due to injury, disease, or surgery in a home care environment.
Execute an appropriate care plan for restorative and maintenance patients.
Collaborate with physicians, case managers, nurses, occupational therapists, physical therapists, home health aides, and office staff to ensure consistent levels of care.
Monitor patient progress and document accordingly.
Educate patients, families, and caregivers regarding the proper use of medical equipment or adaptive equipment.
