JOB RESPONSIBILITIES AND ESSENTIAL FUNCTIONS :
Psychiatric Nurse Responsibilities and Essential Functions :
1. Provides skilled nursing services that promote consumer recovery within the parameters of professional nursing practice. This may include obtaining and documenting a clients height, weight, blood pressure, BMI, or other symptom related communication, including medication education and / or compliance at each visit.
2. Comply with all safety and health standards, rules, regulations, policies and procedures.
3. Participate with other health care providers in the development and modification of a plan of care.
CCBHC Care Coordination Responsibilities and Essential Functions :
1. Monitors consumer compliance with Certified Community Behavioral Health Center (CCBHC) data outcome measures.
2. Consults with consumer and multi- disciplinarytreatment team about treatment options and preferences.
3. Monitors progress of the person-centered or family centered Integrated Treatment Plan for all Psychiatric CCBHC consumers.
4. Coordinates care (routine, urgent, crisis) among primary healthcare, behavioral healthcare, dental, pharmacy, and specialty services inside and outside the agency, including, but not limited to :
a. Scheduling and monitoring appointments
b. Scheduling follow-up services and post-hospitalization appointments
c. Lab work
d. Prescription prior authorization, appeals, adherence
e. Secure releases of information from past or current providers andobtains relevant information
5. Facilitates and monitors internal and external treatment referrals.
6. Monitors progress in achieving goals to demonstrate improvement in the care and clinical outcome of the consumers receiving Psychiatric Nurse Care Coordination services.
7. Complete all clinical documentation related to the position within compliance with regulatory and agency requirements.
JOB QUALIFICATIONS AND REQUIREMENTS :
1. Individual must be currently licensed to practice and in good standing as an RN or LPN in the State of Minnesota.
2. Must have one of the following : 30 semester hours or 45 quarter hours in behavioral sciences or related fields OR Bachelor or masters degree in Health and Human Services related field OR 4,000 hours of supervised experience (without coursework) in the delivery of services to adults or children with : mental illness, substance use disorder, or emotional disturbance; or traumatic brain injury or developmental disabilities and completes training on mental illness, recovery from mental illness, mental health de-escalation techniques, co-occurring mental illness and substance abuse, and psychotropic medications and side effects;
3. Knowledge of and ability to work effectively with persons who experience serious and persistent mental health problems. Knowledge of mental health service delivery and case management principles with a treatment service continuum.
4. Knowledge of Federal, State and County laws and regulations which impact upon service delivery.
5. Ability to work successfully independently and as part of each consumers multi-disciplinary team.
6. Ability to work with diverse populations including age, gender, ethnicity, and cultural components of CCBHC consumers
7. Skilled in planning, problem solving and decision-making. Must be highly organized and creative with an ability to locate resources for clients as needed.
8. Exceptional organization and communication skills (oral and written).
9. Must be able to maintain confidentiality.
10. Must hold a current certificate in CPR / AED / First Aid or be willing to obtain.
11. Must possess a vehicle valid drivers license and a willingness to travel as needed to client or organization locations throughout SE MN.
PHYSICAL REQUIREMENTS FOR POSITION :
Must be able to walk, sit, stand, crouch, twist, bend, reach overhead, maneuver stairs and stretch in a manner conducive to the execution of daily activities. Must be able to move around the office and community settings on a daily basis. While performing the duties of this job, the employee must communicate with others and exchange information. Employee regularly operates a computer and other office equipment on a daily basis. Occasional bending and lifting up to 30 pounds.
NON-ESSENTIAL FUNCTIONS : Perform other duties as assigned by the Program Director.
EQUIPMENT USED : Computer, telephone, office equipment, blood pressure cuff, and vehicle.
