Job Title: Qualified Mental Health Professional Mobile Crisis and Community Stabilization (QMHP)
Location: Virginia
Reports to: Clinical Supervisor
Job Summary:
Hidden Roots Mental Health is seeking an experienced and dedicated team member to fulfill the role of Qualified Mental Health Professional (QMHP). The QMHP will provide individualized training and support to enable individuals to achieve and maintain community stability and independence in the most appropriate, least restrictive environment. This role is integral to ensuring that our clients receive the highest quality care in line with our mission.
Duties and Responsibilities:
- Implement Individualized Service Plans (ISP) based on client needs.
- Provide family relationship-building and parenting skills training.
- Deliver case management services as needed.
- Provide training on activities of daily living as detailed in the ISP.
- Submit all required assessments, reports, and service plans on time, using agency formats that meet client and referring agency needs.
- Coordinate with various agencies in accordance with case management principles.
- Maintain regular contact with referring agencies.
- Document clients’ medication regimens and ensure compliance with appointments.
- Maintain timely and accurate documentation of service provision.
- Manage a small caseload of clients and families.
- Gather clinical information for clients being admitted using all tools provided in Noteable.
- Complete all required admission documents.
- Complete registration requests for Crisis and CSRA for MHSS.
- Report all incidents to the Director of Clinical Services and Program Manager.
- Report directly to the Clinical Supervisor regarding all clinical matters, including service delivery and client progress.
- Function as a mandated reporter.
- Provide weekly schedules/availability to the Administrative Team.
- Ensure that staff maintain a high level of ethical and professional standards.
- Assist with improving healthcare services and communication among clients and staff.
- Review all documentation (ISPs, quarterlies, progress notes, etc.).
- Complete intake and preliminary assessments as needed.
- Support staff in efforts to serve patients effectively.
- Assist workers in obtaining additional education, certificates, and career advancement opportunities.
- Attend interdepartmental meetings and bring quality assurance and risk management issues to appropriate management.
- Provide information and referral services.
- Conduct crisis intervention.
- Provide individual and group supervision to QMHP staff.
- Ensure care coordination.
Minimum Knowledge, Skills, and Abilities:
- Strong knowledge of community-based mental health residential support services.
- Understanding of casework documentation and regulatory compliance.
- Familiarity with mental illness and therapeutic interventions for children, adolescents, and families.
- Knowledge of community resources and entitlement benefits.
- Ability to interact effectively with referral sources and community service providers.
- Competence in utilizing clinical supervision to improve service delivery and documentation.
- Ability to develop and implement person-centered plans.
- Completion of training in CPR & First Aid and behavioral management techniques.
Education and Experience Requirements:
Must be currently registered to practice as a Qualified Mental Health Professional (QMHP) by the Board of Counseling under the Department of Health Professions in Virginia. The following are the specific requirements for different QMHP designations:
1. Qualified Mental Health Professional-Adult/Child (QMHP):
- Master’s Degree in a relevant field: Submit a transcript and evidence of an internship/practicum with at least 500 hours of experience with individuals with mental illness.
- Bachelor’s Degree in a human services-related field: Submit an official transcript and evidence of 1,500 hours of supervised experience within the past 5 years.
- Bachelor’s Degree in an unrelated field: Submit an official transcript, 15 semester credits in a human services field, and evidence of 3,000 hours of supervised experience within the past 5 years.
- Registered Nurse in Virginia: Submit evidence of 1,500 hours of supervised experience within the past 5 years.
- Occupational Therapist in Virginia: Submit evidence of 1,500 hours of supervised experience within the past 5 years.
2. Qualified Mental Health Professional-Trainee (QMHP-T):
- Currently pursuing a degree in a human services-related field and meeting specific criteria set by the Virginia Board of Counseling.
3. Qualified Paraprofessional in Mental Health (QPPMH):
- High school diploma or equivalent and relevant experience as defined by the Virginia Board of Counseling.
Experience:
- Must pass a criminal background and central registry check.
- Maintain a valid driver’s license.
- Complete a minimum of 15 hours of training specific to mental health support services annually.
