Job Overview
The Case Manager will be responsible for providing support and guidance to individuals in need, ensuring they receive the necessary services and resources to improve their quality of life.
Scope of work
- Conduct assessments to determine clients' needs and develop individualized care plans
- Coordinate and monitor services to ensure clients' needs are met effectively
- Provide crisis intervention and support during challenging situations
- Guide and supervise clients to promote personal growth and development
- Utilize motivational interviewing techniques to empower clients towards positive change
- Collaborate with community resources and agencies to enhance service delivery
- Assist with workforce development initiatives for clients seeking employment opportunities
What we are looking for
- Must be open to full time employment and hybrid setup.
- Available to work after school hours and weekends as needed
- Experience in child welfare, childcare, addiction counseling, or related fields
- Knowledge of Medicare regulations and services
- Strong crisis management and intervention skills
- Ability to supervise and mentor individuals effectively
- Must have reliable transportation, drivers license and insurance
- Must have high school diploma or GED. (will be verified)
