RN Clinical Manager- Home Health

High Plains Senior care is currently looking for a RN clinical manager for our Lubbock office. This person will need to be organized and have managerial experience. Home Health experience is required for this position. Home Care Home base experience is a plus. High Plains Senior Care started in 2013 and has since expanded to 8 locations in the state of Texas. RESPONSIBILITIES:

Performs skilled nursing services under the supervision of a staff DON, ADON. In a office setting, observes, records, and reports to the supervisor on the general physical and mental conditions of the patient. Prepares clinical and/or progress notes and incorporates them into the clinical record. Reports to the patient's physician and other appropriate personnel pertinent changes in the patient's condition. Oasis review for accuracy. Process workflow in HCHB. Participates in daily verification's, approve daily interim orders/ review oasis warnings daily. Check hold list daily, prepare weekly case conference review 485/ lock oasis in proper time. Scheduling. Assign PRN/ coordinate admissions. Triage phone and all Dr calls, insurance authorization, Oversee team compliance/ accountability. Check emails frequently and respond promptly. Participates in staff conference regarding patient care.'
