Job Summary:
The Behavioral Health Care Coordinator (BHCC) plays a critical role in supporting patients with behavioral health needs by coordinating care between providers, community resources, and social services. The BHCC ensures that patients receive timely access to integrated behavioral health and primary care services. This position involves patient engagement, case management, and collaboration with a multidisciplinary team to improve patient outcomes.
Key Responsibilities:
- Coordinate behavioral health services for patients, including scheduling appointments, referrals, and follow-ups.
- Assist in developing and implementing patient-centered care plans in collaboration with medical and behavioral health providers.
- Provide patient education on mental health conditions, treatment options, and available community resources.
- Conduct outreach to patients for follow-ups, medication adherence, and engagement in treatment plans.
- Facilitate communication between patients, primary care providers, behavioral health clinicians, and external agencies.
- Track patient progress, document care coordination activities, and maintain accurate records in the electronic health record (EHR) system.
- Support crisis intervention and assist in linking patients to emergency mental health services when necessary.
- Collaborate with social workers and case managers to address social determinants of health that impact mental wellness.
- Advocate for patients' needs within the healthcare system and assist with navigation of insurance coverage and benefits, including Medi-Cal and Medicare.
- Ensure compliance with all applicable federal, state, and local regulations, including HIPAA and FQHC-specific guidelines.
Qualifications:
- Bachelor's degree in social work, psychology, public health, or a related field, preferred.
- Minimum of two years of experience in care coordination, case management, or behavioral health services, preferably in an FQHC or community health setting.
- Familiarity with Medi-Cal, Medicare, and other healthcare coverage programs.
- Experience working with diverse and underserved populations, including individuals with serious mental illness (SMI) and substance use disorders (SUD).
- Strong knowledge of behavioral health resources and community-based services.
- Proficiency in electronic health record (EHR) systems and Microsoft Office Suite.
- Excellent interpersonal, communication, and problem-solving skills.
- Ability to work independently and collaboratively within a multidisciplinary team.

Language Ability:
Must have intermediate level language skills. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Math Ability:
Must have basic math skills. Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentages.
Reasoning Ability:
Must have intermediate level reasoning skills. Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
Computer Skills:
To perform this job successfully, an individual should have good working knowledge of Windows operating systems, Microsoft Office Suite Word, Excel, Outlook, and Internet browsers and related software. Must have good working knowledge of medical management software. Proficient with computers, copiers, fax machines, printers, multi-line telephone systems.
