What are the responsibilities and job description for the Medical Assistant position at Hillside Medical Office?
Duties & Responsibilities:
1. Patient Intake:
- Escorts patients from the reception area to exam rooms and/or other clinic areas.
- Obtains vital signs and chief complaints.
- Obtains and/or reviews family history.
- Screens patients for self-referrals and following up to obtain reports and/or test results.
2. Documentation:
- Conduct pre-visit preparations, including but not limited to, chart review for outstanding orders, outstanding test results or referrals, preventive medicine needs, education needs, etc.
- Enter Clinical information in EHR--vitals, problem lists, medications (including OTC), allergies, growth charts, immunizations, tobacco use, medical/family/social history, communication needs, behaviors affecting mental health, etc.—in accordance with documentation guidelines established by the practice and meaningful use measures established by CMS.
3. Patient Care:
- Opens and closes areas, including phones, computers and applicable programs, exam rooms, equipment and paperwork
- Administers P.O., and I.M. injections and medications as ordered (after completion of competency training) and documents administration in medical record.
- Performs direct patient care within the limits of his/her training and capabilities when the provider is unavailable.
- Prepares patients for examination, treatments and procedures including preparation of room and/or equipment.
- Chaperones and assists providers during exams and procedures.
- Maintains timely flow of patients and informs patients in the clinic area of appointment delays; coordinating with front office staff as indicated.
- Utilize standing orders established by the practice for preventive and chronic care conditions in delivering patient care and education.
- Review and reconcile medications with the patient and/or their caregiver(s).
- Assesses patient/care giver understanding of medications.
- Assess patient response to medications and barriers to adherence.
- Assess and document barriers the patient and/or their caregiver(s) may be encountering in their efforts to improve their health.
4. Patient Communications:
- Returns or receives patient phone calls during office hours; performs triage of patients by phone or in office, assessing medical needs of patients and coordinating appropriate care or interventions and referring to licensed staff or providers as indicated.
- Return calls to patients within specified timeframes established by practice.
- Return electronic messages to patients within specified timeframes established by practice.
- Document clinical advice within the electronic medical record within timeframes established by practice and in accordance with documentation guidelines and medical standards of care.
- Notify patients of test results per office protocol.
- Provide clinical summaries to patients at the end of appropriate patient encounters.
- Provide patient education and self-management resources to patients and/or their caregiver(s).
- Provide advanced directives education/resources to patients and/or their caregiver(s) and documents status in the EHR.
- Utilize motivational interviewing techniques to assist patients and/or their caregiver(s) with setting goals to improve their health and to self-manage their care.
5. Lab and Imaging:
- Performs CLIA waived lab tests.
- Assures proper labeling and handling of all specimens. Completes appropriate requisitions and/or paperwork.
- Maintains a follow-up system for lab results.
- Coordinate scheduling lab and imaging procedures performed outside the practice.
- Flag abnormal test results and bring to the attention of the provider.
- Notifies patients of their results subsequent to provider review.
- Educates and prepares patients for exams; provides emotional support and reassurance as needed.
- Monitor outstanding lab and imaging reports for past due test results and follow-up with the appropriate facility to obtain the result in accordance with timeframes established by the practice.
6. Care Coordination:
- Coordinate prescription renewals via phone and/or electronic health record and document in accordance with documentation guidelines and medical standards of care.
- Coordinates referrals and schedules appointments with specialists and ancillary services on behalf of patients in the Patient Care Coordinator’s absence.
- Coordinate scheduling appointments with specialty providers, including substance abuse and/or mental health.
- Monitor outstanding referrals for past due consult reports and follow-up with the appropriate provider office to obtain the result in accordance with timeframes established by the practice.
- Monitor hospital admissions and emergency room encounters at facilities the practice has established reporting access with and ensures receipt of discharge summaries and other applicable records.
- Contact patients proactively to schedule follow-up visits for patients that have had a hospital admission or emergency room visit.
- Identify patients that have had a hospital admission or emergency room visit and request records.
- Forward summary of care documents and copies of applicable patient records to hospitals and/or emergency departments while patients are receiving care.
7. Quality Improvement:
- Maintains clinical equipment, supply inventory, and stocks exam rooms.
- Cleans and sterilizes exam rooms, equipment and instruments.
- Maintains patient confidentiality at all times.
- Participate in daily huddles with the assigned care team, department and/or all staff meetings.
- Attend special training sessions based on assigned job duties.
- Develops and documents protocols related to his or her job functions.
- Participate in the clinic's quality improvement activities by participating in PDSA groups, providing feedback, reporting incidents, assisting with workflow redesign, collecting and reporting assigned performance indicators, and Quality Improvement Plan (QIP) review and evaluation activities.
8. Performs other appropriate duties as requested by supervisors.

Education and Experience:
- Prefer graduate from an accredited program for CMA, CNA, or similar certification.
