Hourly Rate: $64.23 to $71.90
Position Overview:
We are seeking a Case Manager/Discharge Planner RN to join our team for a 13-week contract. This position is perfect for a compassionate, experienced RN with expertise in discharge planning and case management. You will play a vital role in ensuring safe and efficient patient discharge processes while collaborating with the healthcare team.
Key Responsibilities:
- Conduct patient assessments to determine discharge needs
- Collaborate with healthcare providers to create effective discharge plans
- Manage utilization reviews and coordinate patient care transitions
- Review insurance benefits and covered services (Medicare/Medicaid preferred)
- Ensure compliance with hospital policies and regulations
- Provide education and support to patients and families about post-discharge care
Qualifications:
- Current, unencumbered Oregon RN Licensure required (BSN preferred)
- Minimum of 3 years of RN experience required
- Healthcare Provider Level BLS certification required
