Role: Nurse Practitioner (Adult/Geriatric)

Job Type: Part-Time (Onsite)
Shift: 3 Shifts per Week | 5 Hours per Day
Role Overview:
We are seeking a dedicated Nurse Practitioner (NP) to provide comprehensive primary care to residents in assisted living facilities. This role offers a rewarding opportunity to collaborate with an interdisciplinary team and enhance the quality of life for older adults in long-term care settings.
Key Responsibilities:
- Conduct health assessments, including physical exams, medical history reviews, and medication reconciliation.
- Develop and implement personalized care plans in collaboration with the healthcare team.
- Manage chronic conditions and acute illnesses through appropriate interventions.
- Order and interpret diagnostic tests (e.g., lab work, imaging) for accurate diagnosis.
- Prescribe medications and treatments within the NP scope of practice.
- Provide chronic care management services to optimize patient health outcomes.
- Educate and counsel residents and families on disease prevention, health maintenance, and self-care.
- Document assessments, treatments, and communications using eClinicalWorks (eCW).
- Participate in quality improvement initiatives and interdisciplinary meetings.
Requirements:
- Master's Degree in Nursing and completion of an accredited Nurse Practitioner (NP) program.
- Unrestricted NY State Nurse Practitioner License in good standing.
- Active DEA Permit to prescribe medications.
- Medicare and Medicaid Provider Status eligibility.
- Minimum 3 years of clinical experience (3600 hours required), preferably in long-term care.
- Primary care experience is preferred.
Preferred Qualifications:
- Experience with geriatric populations in assisted living or long-term care settings.
- Proficiency with eClinicalWorks (eCW) or similar EHR systems.
- Strong clinical decision-making and a patient-centered care approach.
- Excellent communication and collaboration skills.
