Key Responsibilities:
Health Education & Promotion:
- Provide culturally appropriate health education on topics such as disease prevention, nutrition, and chronic disease management.
- Conduct outreach programs and workshops to raise awareness about health issues. Encourage individuals to adopt healthy behaviors and access preventive care services. •
Community Outreach & Engagement:
- Serve as a liaison between healthcare providers, social services, and community members.
- Build trust and relationships with individuals and families to improve access to care. Identify community health needs and advocate for necessary resources and services.
Data Collection & Reporting:
- Document interactions and maintain records of client progress and service referrals.
