EDUCATION/TRAINING:
• Registered Nurse currently licensed by the State of New Jersey
• CPR Certification

• Ability to anticipate the needs and moods of residents and interact with residents under all conditions

• Ability to read, write, speak, understand and communicate in English sufficiently to perform the duties of this position
EXPERIENCE:
• Experience in Long Term Care nursing preferred

• Assume charge of unit in absence of supervisor, if necessary

• Ensure all assigned user ID codes and passwords for accessing resident / facility information are kept confidential; ensure the departmental computer workstations left unattended are properly logged off or the password protected automatic screen-saver activates within established facility policy guidelines
• Report known or suspected incidents of fraud to the Administrator
• Report any known or suspected unauthorized attempt to access facility’s information system
• Maintain HIPAA compliance as well as confidentiality of all data, including resident, employee and operations data
• Exhibit concern for safety and well-being of staff, residents, families, and visitors
• Perform standard assessments
• Protect residents and employees by adhering to infection control policies and protocols, medication administration and storage procedures, and controlled substance regulations
ESSENTIAL DUTIES:
• Observe, assess and report changes in resident’s condition to immediate supervisor, attending physician, family or designated contact person
• Call or accept verbal orders from resident’s physician and transcribe them according to policies and procedures
• Be present for shift change and transfer or residents condition by the previous shift charge nurse
• Assure quality of care by adhering to therapeutic standards; measuring health outcomes against resident care goals and standards; make or recommend necessary adjustment to resident plan of care with their input
• Administer medications and treatments from medication cart, which is moved down the hall to each resident’s room. Follow physician’s orders, which are on the medication administration record. Initial M.A.R. sheets
• Resolve resident issues and needs by utilizing multidisciplinary team approach/strategies
• Document appropriately and report any drug or treatment errors to supervisor or physician; discuss total plans of care and problem list with the MDS coordinator
• Oversee LPNs and CNAs
• Allocate staff assignments for CNAs in the absence of supervisor
• Document on resident’s chart relative to policy and maintain clinical records in a prescribed manner
• Use appropriate and safe body technique when lifting and/or transferring residents or supplies
• Operate equipment required for resident care
• Use mechanical equipment and assistive devices for patient handling tasks as identified, according to the individual resident Care Plan and the Safe Patient Handling Act.
• Assist physician during examination and/or treatment of resident if needed. Review orders with physician
• Assist in training and supervising CNAs
• Attend scheduled meetings and continuing education programs to keep abreast of current trends in medical science
• Understand and apply basic principles of safety
• Participate in the Quality Assessment Performance Improvement Program (QAPI)
o Attend trainings to build understanding and capacity to undertake QAPI work
o Carry out QAPI roles and responsibilities as assigned
o Follow established policies and procedures in support of QAPI efforts
o Collect required data for Quality Assurance and Performance Improvement (QAPI) purposes, as directed
OTHER DUTIES:
• Timely completion of all required in-services
• Must be present for at least one mandatory annual fire drill
• Must participate in annual N95 Fit Testing
• All other responsibilities that may be necessary to fulfill the duties associated with the position or as may be assigned
HOURS OF WORK:
• As per department schedule
• Weekends, if required
