Job Description :
- Social workers will be assigned to various units primarily including oncology, med / surg, and ICU.
- Conduct assessments and develop discharge planning plans for patients with diverse needs such as nursing home placements, home health services, and support for homelessness and crisis situations.
- Address the needs of uninsured patients and other complex cases.
- Work every other weekend and may occasionally cover shifts at a sister hospital.
- Collaborate with multidisciplinary teams to ensure comprehensive care management.
