LCSW Social Worker masters prepared with Maryland license prior to interview.
Must have 3 -5 years of recent acute discharge planning experience in the inpatient hospital setting.
The social workers will be assigned to various units primarily; oncology, med/surg, and ICU.
Will work every other weekend.
Required to do assessments and discharge planning plans for patients requiring all types of needs varying from nursing home placements, home health services, dealing with homelessness, crisis patients, uninsured patients, etc.
Rarely could be asked to cover shifts at sister hospital located in Germantown.
