What are the responsibilities and job description for the Hospice Chaplain/Bereavement Coordinator position at Holy Savior Hospice?
JOB SUMMARY:
Primary function is to provide spiritual care in the home to patients of all age groups and perform spiritual assessments, analysis, counseling and referrals to meet the needs of the patient/family.
TRAVEL REQUIREMENTS:
Holy Savior Hospice services Corpus Christi and surrounding cities to include: Falfurrias, Kingsville, Robstown, Rockport, Portland, Ingleside, Taft, Sinton, George West, and Alice.
ESSENTIAL DUTIES:
- Models a service attitude towards others, takes ownership in solving problems and takes the initiative to make things better, is friendly and courteous to residents, patients and co-workers
- Ensure resident/patient, visitor, and co-worker safety by remaining aware of and reporting any new or existing safety hazards
- Maintain good attendance record
- Demonstrates appropriate assessment skills for initial and continued spiritual evaluations and assists in the development and implementation of goal-directed family care plan.
- Actively participate in coordination of all aspects of patient s care, in accordance with current professional standards and practice
- Participate in ongoing interdisciplinary comprehensive assessment
- Develop and evaluate the plan of care
- Contribute to patient and family counseling and education
- Participate in QAPI program and hospice sponsored in-service training
- Conducts ongoing reassessments and follow-up appraisal of patient/family situation and needs. Actively listens and responds appropriately to patient/family concerns and needs in a timely manner.
- Assumes the active role of advocate for the patient/family.
- Provides spiritual care to staff as needed
- Possesses an adequate knowledge of community resources.
- Develops and maintains positive working relationships with religious/spiritual community personnel.
- Works under the direction of the Patient Care Manager and Director of Spiritual Care to provide direct spiritual care to patients and families to ease, resolve, or reconcile spiritual pain and distress precipitated by life-limiting illness
- Provides spiritual care to staff as needed
- Ensures that all patients/families under his/her direct supervision are provided care and services to attain or maintain the highest practicable physical, mental, psychosocial and spiritual well-being considering each individual patient/family.
- Treats patient/family with compassion, adapting communication to meet situations and cross cultural issues.
- Supports patient and family sense of control, while striving to reduce their anxiety.
- Completes all assigned duties as scheduled. Submits documentation on time.
- Participates in interdisciplinary team meetings.
- Refers questions outside area of expertise to appropriate team members or community agencies.
- Perform occasional liturgical duties such as Hospice Memorial Service.
- Conduct or make arrangements for funeral or memorial services for patients when indicated.
- Arrange for on-call availability of services as directed.
- Provide educational programs for community clergy, religious and lay representatives as resources allow.
- Other duties as assigned by the Patient Care Manager or Director of Spiritual Care
MINIMUM QUALIFICATIONS:
- Graduate of accredited seminary or school of theology
- Credentialed by an ordaining or licensing religious or spiritual body
- Completion of 4 units CPE residency or 3 units plus equivalency
- Willingness to pursue APC certification
- Valid Texas Drivers License and reliable method of transportation
SALARY / PAY:
Mileage: $0.50/ mile
Annual salary range: $58,000 - $65,000
On call pay: $2/ hr
On call visits: $35
On call death visit: $100
