NURSE CARE NAVIGATOR

HOMAHA SENIOR CARE, LLC is seeking a compassionate, energetic LPN to establish a professional and genuine rapport with community members. Working side by side with an experienced Care Navigator, the successful candidate witll provide clinic-like medical care in a Senior Living community during regular business hours Monday-Friday, no weekends or on-call.
CARE NAVIGATOR PROGRAM OVERVIEW The Care Navigator Program is your one stop source for coordination of appointments, home health care services transportation and much more.
ONE HOUR RESPONSE In minor emergencies, timely response may be the difference between treating at home or going to the Emergency Room of a hospital. Our providers are available for urgent virtual visits to immediately address symptoms.
IN HOME SERVICES If labs or x-rays are needed, there is no need to leave home. Our providers can arrange for these and more services to be done where our clients reside – eliminating the need for transportation and saving time and hassle.
BACK UP PROVIDER With virtual visits and routine checks via our physician and nurse practitioners, our program supports your Primary Care Physician in treating your health concerns.
REQUIREMENTS:
- ACTIVE, IN GOOD STANDING LPN/LVP or RN LICENSE
- 1 YEARS OF GERIATRIC EXPERIENCE (PREFERRED)
- SELF-MANAGED, SELF STARTER, EAGER TO LEARN ATTITUDE
COMPENSATION: 55-65K
