Home Care Now is hiring LPN's for Medicare/Skilled Visits for PER DIEM positions serving patients in Pinellas county.
Must have HOME CARE EXPERIENCE / MUST HAVE MEDICARE KNOWLEDGE performing patient visits in the home. Pay range is $35 - $50/visit. Set your own schedule. EMR System is Axxess.

Responsibilities
- Perform patient care following the physician's plan of care and orders
- Accurately report and record the patient's condition and care, including signs and symptoms which may be indicative of change
- Prepare clinical/progress notes and all required documentation meeting Medicare's standards
- Perform all duties and responsibilities always adhering to Florida chapter 464 - Nurse Practice Act
Qualifications
- Graduate of an accredited Diploma, Associate, or Bachelor’s School of Nursing, or vocational nursing
- Current state license as an LPN in good standing
- Current CPR certification
- Valid driver’s license and reliable transportation
- At least ONE year experience as an LPN at a Medicare Home Health Agency
- Medicare documentation experience required
