Quality Assurance Nurse, Remote
Home Care Services of Northwest Indiana is seeking multiple Quality Assurance Nurses to add to our home health team in Michigan City, IN. We’re looking for QA Nurses to be the advocate for both our home bound patients and our home health organization. Our QA Nurses function as the liaison between the patient, provider, and payer. This person will commonly perform audits, inspections, and other assessments of our home health services to ensure that we meet all relevant requirements.
Requirements:
· Graduate of nursing school and currently holding an LPN license.
· 1 year of previous nursing experience, preferred.
· Work closely with the Director of Nursing and Field Staff regarding their corrections and that documentation is submitted in a timely manner, properly coded, and meets a quality standard.
· Ability to communicate effectively.
· Strong ability to use critical thinking and deduction when performing job tasks.
· Ability to review clinical records for consistency, compliance with regulation, quality of documentation, coordination of the multidisciplinary plan of care, and clinical outcomes.
· Meticulous accuracy.
Job Responsibilities:
· Complete clinical record review at the start of care, resumption of care, recertification, and discharge for complete and accurate documentation, development of appropriate discipline-specific care plan, accurate physician orders, and coordination of care and compliance with Medicare and state regulations.
· Completes all documentational reviews within agency-defined deadlines.
· Demonstrates understanding of current Medicare guidelines and ensures that clinical records are compliant with COPs; maintains agency in survey-ready status.
· Demonstrates proficiency in use of the EMR and other web-based programs.
· Provides expertise in OASIS, records review, participates in staff education and onboarding programs.
