Schedule : Monday - Friday (Full-Time) rotating weekends

Home Health Care, Inc. and Summit Home Care are two community-focused home health agencies, each providing essential services to help individuals live safely and comfortably in their own homes. We are seeking a Home Health Unit Coordinator / Intake Coordinator to join our Golden Valley team! This role is pivotal in managing the intake and admission process for new patients across multiple lines of business. You will help ensure that our clinical team, reimbursement processes, and schedules align with each client’s needs, making a positive impact on our communities.
