Sunflower Communities, an assisted living facility located in a beautiful rural setting, is seeking a Clinical Assessment Nurse to join our team. As a Clinical Assessment Nurse, you will be responsible for assessing the health needs of our residents and developing individualized care plans to ensure their optimal health and wellbeing. In this role, you will work closely with our interdisciplinary team, including physicians, nurse practitioners, and other healthcare professionals, to provide comprehensive and coordinated care to our residents.
As an Assessment Registered Nurse, you will play a crucial role in ensuring that patients receive high-quality, patient-centered care that is tailored to their individual needs. Your expertise in assessment and care planning will help patients achieve optimal health outcomes and improve their overall quality of life.
Assessment Nurse (RN) Qualifications:
- A current, unrestricted license to practice as a registered nurse (RN) in the state of employment.
- A Bachelor of Science in Nursing (BSN) degree or higher from an accredited nursing program.
- At least 2-3 years of clinical experience in a healthcare setting, with experience in assessment and care planning preferred.
- Excellent communication skills, with the ability to effectively communicate with patients, families, and healthcare professionals.
- Strong critical thinking and problem-solving skills, with the ability to make sound clinical judgments.
- Knowledge of nursing practice, healthcare regulations, and emerging trends in assessment and care planning.
- Ability to work independently and as part of an interdisciplinary team, collaborating with other healthcare professionals to provide comprehensive and coordinated care.
Assessment Nurse (RN) Responsibilities:
- Conducting comprehensive assessments of patients' physical, mental, and emotional health, as well as their social and environmental needs.
- Developing and implementing individualized care plans based on the assessments, in collaboration with the patient, their family, and other healthcare professionals.
- Monitoring and evaluating patients' responses to the care plan, modifying it as necessary to ensure optimal outcomes.
- Communicating with patients, their families, and healthcare professionals to coordinate care and ensure that everyone is informed about the patient's progress.
- Providing education to patients and their families on their health conditions, treatment options, and ways to promote wellness.
- Documenting all assessments, care plans, and interventions accurately and in a timely manner.
