Introduction to the Company
Home & Health Care Management is the oldest home health care agency in Northern California. Our agency specializes in delivering comprehensive in-home care and case management for older adults, children, and those living with disabilities across many counties. We use a matrix of Medi-Cal, Medicare and targeted health programs to find solutions that work for our clients.
Overview of the Role
You are an experienced social work case manager, and are comfortable supervising other case managers. You have the skills to supervises the operations of the Medi-Cal Waiver Program (MCWP) program and team members, as well as reporting on the work of the program. You will report directly to the CEO. You will be supervising team members who are managing a caseload of HIV/AIDs clients.
Responsibilities:
To perform this job you must be able to perform the following duties successfully:
- Understand the Medi-Cal Waiver Program (MCWP) contract, all caseload specifications, and be able to implement the terms of the Agency Contract.
- Grow the MCWP case load to full capacity, and meet caseload deadline
- Develop, monitor and maintain a tracking system to ensure all clients are scheduled for the appropriate contact each calendar month, and report on the program
- Supervise the work of the case managers by reviewing their documents for quality, accuracy, adherence to time frames, and by training case managers in all aspects of the MCWP: policies
- Review and approve Financial Assistance Requests as appropriate.
- Schedule and assign caseload to the case managers
- Ensure case managers are aware of and attend monthly interdisciplinary case conferences, and also attend as many of these conferences as possible.
- Conduct home health visits
Essential Requirements:
- Bachelor’s degree in Social Work, Psychology, Counseling, Rehabilitation or Sociology
- One year experience as a Program Manager or Supervisor
- Excellent verbal, written and critical thinking skills
- Ability to inspire supervised staff and develop / maintain a positive work atmosphere as a leader / teacher
- Valid California Driver’s License and eligible to be insured under our liability policy with a clean DMV report
- Your own vehicle and proof of current auto insurance
PPE is provided to all employees
Preferred Skills and Experience:
- Master’s degree in Social Work, Psychology, Counseling, Rehabilitation or Sociology is preferred
- Three years experience as a Program Manager or Supervisor
- Prior knowledge of HIV / AIDS
- Working knowledge of benefits programs, community programs/services specific to HIV/AIDS clients
- Fluency in Spanish
- Ability to work with others, at all levels within the organization, and collaborate effectively.
- Above-average interpersonal, problem-solving, and written and oral communication skills.
- Professional behavior when on duty
Service Region/ Location:
This is not a remote role and requires you to work in our Chico office.
Schedule:
This is a full-time role, 40 hours per week, from Monday through Friday from 8am to 5pm with an hour for lunch.
Travel:
You will need your own car and be able to travel to client’s homes within Butte County and surrounding counties.
Compensation:
Pay range is $30 to $35 per hour. Exact compensation may vary based on skills, experience, and location.
Conditional upon job offer is successfully passing a pre-employment medical exam, and background check.
Benefits
You will be reimbursed for the mileage you travel from the office to participant’s homes.
You will receive a monthly phone stipend.
On the first of the month following 60 days of employment you will be eligible for health insurance, dental insurance, vision insurance, life insurance, as well as Flexible Spending Account (FSA).
Leave accrues starting on your first day of employment, and you can take paid vacation, sick and holiday leave after 90 days.
We offer a 401(k) matching up to 4% after 1 year of employment and 1,000 hours worked.
