At Mosaic, a unique Adult Day Service program, you'll help provide a balance of medical support and opportunities for fun and skill-building.

Mosaic operates under a medical model, offering comprehensive services such as nursing care, physical integrity, range of motion exercises, and health and hygiene support, all under RN oversight to ensure individualized care. Beyond medical services, Mosaic fosters creativity and joy through activities like arts and crafts, exercise programs, community integration opportunities, and group activities, empowering participants to develop skills, build independence, and form meaningful connections.

What You’ll Do :

  • Oversee the day-to-day operations of Mosaic, combining medical support with engaging activities.
  • Work collaboratively with the Program Manager and RN to provide comprehensive care and services.
  • Supervise and mentor Direct Support Professionals (DSPs) to ensure high-quality support.
  • Develop and implement individual service plans tailored to participants' needs and goals.
  • Coordinate schedules that balance medical needs with creative and skill-building activities.
  • Manage facility resources, equipment, and supplies while maintaining compliance with licensing standards.

What We’re Looking For :

We’re seeking a dedicated professional who brings the following qualifications, skills, and attributes :

  • Education & Experience : A bachelor’s degree in human services or a related field, or equivalent experience in providing direct services to persons with developmental disabilities.
  • Technical Skills : Proficiency in using electronic communication tools and software.
  • Certifications & Clearances : Current First Aid and CPR certification, pre-employment physical, TB screening, and FBI fingerprint clearance.
  • Administrative Skills : Strong ability to maintain accurate records, complete detailed individual service plans, and ensure compliance with all documentation standards.
  • Transportation : Reliable transportation with current insurance (recommended).
  • Health & Safety : Willingness to be tested and inoculated for Hepatitis B (recommended).
  • Workplace Readiness : Must pass a pre-employment drug screening and adhere to established dress standards.
  • Leadership Skills : Ability to supervise and mentor staff, collaborate with stakeholders, and coordinate activities that align with client goals.
  • Age Requirement : Applicants must be 18 years or older.
