Responsibilities
- Completes an initial home health assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
- Utilizes the nursing process in planning, providing and evaluating skilled needs.
- Completes a comprehensive patient assessment that is reflected in accurate OASIS scoring.
- Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and family on the planning and revising process.
- Case management of your patients! You are key to helping them heal, getting back to feeling strong and healthy!
Qualifications
- Graduate of an accredited school of nursing.
- Associate degree in nursing.
- Registered as RN with New Hampshire’s Board of Nursing.
- One years’ recent acute care experience. Preferably in med/surg or acute rehab setting.
- Ability to exercise initiative and independent judgment.
Preferred Qualifications
- Bachelor’s degree in nursing.
- Community Health/Education experience.
- Certifications: IV, Chemotherapy, Ostomy, as examples.
- Familiarity with OASIS and what drives reimbursement.
- EMR software proficiency.
Required
- Registered as a RN with New Hampshire’s Board of Nursing.
- CPR Certification.
- Satisfactory criminal record.
- Satisfactory motor vehicle record.
