Home Plus LPN

Job Summary:
Currently seeking a Full Time LPN, to include weekends. The LPN will assist patient care at our Home Plus houses. The LPN would be available to do rounds, provide patient care to assist the CNA/CMA team and be responsible for providing nursing care needs as requested and report any additional needs to the House Nurse Operator and Administrator.

Responsibilities:

- Report to and assess patient needs within LPN Scope

- Administer and record medications as needed

- Provide house rounds and team collaboration for positive patient outcomes.
- Monitor patients' conditions and report any changes or concerns to the healthcare team.
- Manage pain and discomfort for patients, using a variety of techniques and medications.
- Work in the emergency room, assisting with urgent care and acute patient needs.
- Collaborate with physicians, registered nurses, and other healthcare professionals to ensure the best possible outcomes for patients.
- Maintain accurate and detailed patient records, including vitals, treatments, and medications.
- Stay current with the latest medical protocols and standards.

Qualifications:

- Current licensure as a Licensed Practical Nurse (LPN) in the state of employment.
- Strong understanding of medical-surgical procedures and pain management techniques.
- Ability to work effectively in a team setting.
- Excellent communication and interpersonal skills.
- Ability to maintain patient confidentiality and adhere to HIPAA regulations.
