Responsibilities
- Conduct comprehensive diagnostic evaluations to assess patient needs and develop individualized care plans.
- Manage and coordinate all aspects of patient care, including communication with healthcare providers and family members.
- Monitor vital signs and overall health status, making necessary adjustments to care plans as required.
- Provide education and support to patients and families regarding health management and treatment options.
- Facilitate discharge planning for patients transitioning from inpatient settings to home health care.
- Collaborate with interdisciplinary teams to ensure comprehensive patient support.
- Maintain accurate documentation of patient progress and interventions in compliance with healthcare regulations.
- Stay updated on best practices in geriatrics and managed care to enhance service delivery.
Qualifications
- Valid Registered Nurse (RN) license in Kansas.
- Proven experience in case management or home health nursing, preferably with geriatric populations.

- Knowledge of physiology and ability to assess vital signs accurately.
- Experience in discharge planning processes is highly desirable.
