What are the responsibilities and job description for the Targeted Case Manager position at Home Helpers Home Care?
Home Helpers Home Care is now hiring for a Targeted Case Manager for our Mena and Nashville offices. This position primarily operates remotely with the expectation of monthly in-person visits to clients' homes.
The Case Manager will be responsible for coordinating care, assisting clients with obtaining necessary equipment and supplies, educating them on local services, and closely monitoring their progress to ensure the highest level of support and care.
Key Responsibilities: Home Visits, Documentation, Coordination of services, Education and advocacy, monitoring progress, and collaboration.
Flexible, Full Time Position.
Bachelor's degree in health science, LPN, RN, or 2 years experience in Case management required.
Job Type: Full-time
Pay: $15.00 - $17.00 per hour
Expected hours: 40 per week
