Summary
As a Qualified Intellectual Disabilities Professional at Home of Hope Inc, you will play a vital role in supporting individuals with disabilities to achieve their personal goals and enhance their quality of life. Responsibilities
- Develop and implement individualized support plans tailored to the needs of each client.
- Provide direct support and assistance to individuals in daily living activities.
- Facilitate community integration by organizing and participating in social and recreational activities.
- Monitor clients' progress towards goals, adjusting plans as necessary to ensure effectiveness.
- Collaborate with families, caregivers, and other professionals to enhance the support network for clients.
- Maintain accurate documentation of client interactions, progress notes, and incident reports.
- Ensure compliance with all relevant regulations and standards in service delivery.
- Promote a positive environment that encourages independence and self-advocacy among clients.
Qualifications
- Bachelor's degree in social work, psychology, or a related field preferred.
- Experience in direct support or caregiving roles for individuals with disabilities is essential.
- Strong interpersonal skills with the ability to build rapport with clients and families.
- Knowledge of best practices in disability services and person-centered planning.
- Ability to lift and assist clients as needed; physical stamina is required.
- First aid certification is a plus but not mandatory upon hiring.
