SUMMARY:
The Facility Navigator role refers to supporting staff that will become the main point of contact between the designated facility (Skilled Nursing Facility) and Home Physicians Group.
QUALIFICATIONS & ATTRIBUTES:
- High school diploma or GED.

Medical Assistant Certification with license number or experience:
- 1 year of previous medical assistance experience.
- 2 years of previous medical assistance experience if uncertified.
- Post high school education is advantageous.

Proven experience working in a medical setting including knowledge of:
- Medical Terminology
- EMR systems
- Basic pharmacology
- Proficient in Microsoft Office applications (Word, Excel, Outlook).
- The ability to type and file accurately.
- Strong communication, interpersonal and presentation skills.
- Good computer and electronic record skills.
- Excellent organizational skills.
- Aptitude in problem-solving, critical thinking, and decision-making.
- Outstanding time management skills.
- Excellent written and verbal communication skills.
- Strategic thinking and analytical skills.
MAJOR AREAS OF RESPONSIBILITY:
- Live and practice the Mission of HPG by providing quality patient care.
- Provide quality customer service and communicate on a regular basis with the designated facility staff.
- Maintain a healthy professional relationship with clinical management and rounding providers.
- Communicate on a regular basis with clinical management and rounding providers.
- Ability to properly communicate with schedulers to ensure patients clinical status are up to date and providers visits are appropriate for their encounter types and reasons for visits (as needed).
- Ability to properly communicate with medical record specialists and case management in order to comply with patient’s record updates and continuity of care.

Ensures proper documentation for patients’ visits, including but not limited to completing the face-sheet portion, which includes:
- Current medications
- Past medical history
- Social History
- Surgical History
- Allergies
- Code Status
- Vitals
- ICD-10 codes and diagnosis
- Assist providers in conducting Annual Wellness Exam and MIPS visits questionnaires.
- Ability to utilize multiple EMR systems, including medical AI programs, in order to record the most accurate documentation.
- Demonstrates adherence to HIPAA confidentiality standards.
- Coordinates and establishes a professional relationship with facility staff and patients to properly capture patient’s demographic, insurance data, health records, diagnostic results, and any pertinent information.
- Ensure all patients’ needs are addressed by our medical providers.

Communicates and establishes interpersonal relationships with patients, patients’ families and LOAs.
- Communicate basic clinical information to staff and patients.
- Updates patients’ health record on a regular basis, including diagnostic results, clinical reports, medication changes, etc.
- Ability to capture vital signs as needed.
- Follow up with proper prescription refills procedures.
- Ability to monitor and follow up patient medical condition changes, by recognizing and reporting acute abnormal findings.
- Ensure all pertinent information including diagnostic results and reports are available for providers to review.
- Ability to recognize and promptly respond to urgent/emergent situations on a proper matter.
- Demonstrates adherence to Chronic Care Management standards and expectations.
- Complete other tasks assigned by manager.
