Summary
The Referral Coordinator (RC) is an hourly role, reporting to the Office Manager. This position is expected to report to the company office daily unless otherwise directed. As a Referral Coordinator, you will perform a wide range administrative duties to include referral processing, patient registration, verification of benefits, insurance authorizations, patient appointment scheduling, program communication to potential candidates, communication with referral sources and other tasks as assigned. You will assist your assigned color team, medical practitioner, and patients. The role of a Referral Coordinator is crucial in ensuring patients receive comprehensive and up-to-date care.

Responding to Microsoft Teams, emails, as well as voicemails left for you daily and all should be addressed and cleared before clocking out for your shift.
Work with assigned In-clinic and weight loss patients.
Responsible to the tracking, coordination, and communication of patient referrals
Handles requests from third party referral sources and provides relevant information and data.
Assembles information concerning patient’s clinical background and referral needs (i.e., lab results, reviewing prior encounters for confirmation of sending/receiving orders etc.)
Contact insurance companies to ensure verification of benefits and prior authorization approval requirements are met.
Review details and expectation of referral with potential patient.
Assists patients in problem solving potential issues related to financial or social barriers (i.e., request interpreters as appropriate, transportation services etc.)
Ensure that referrals are addressed in a timely manner.
Reminds patients of scheduled appointments related to referrals.
Communication with referral sources to ensure patient information/ registration is complete.
Communicates patient updates between program and referring source.
Complete orders given by medical practitioners.
Prepare the schedule for the following day with records.
Ensure the census is properly maintained.
Ensure the workflow process is being completed daily.
Wellness calls and medication reconciliation.
Thorough, accurate documentation of all activities.
Ensure patient has a scheduled follow up PCP appointment for referred appointment.
Contact 3rd parties, such as specialists, hospitals, and home health care agencies to obtain notes for our medical practitioners to review.
Assist with wound care orders.
In clinic MA coverage as needed.
Other duties as assigned at the discretion of Office Manager.
