The Street Outreach Specialist (SOS) is a day shift role who works directly with individuals in the community experiencing unsheltered homelessness. The SOS provides case management, education, harm reduction, and advocacy to help clients connect with community resources, including recovery services, emergency shelter, and permanent housing. Additionally, this position ministers to clients' spiritual needs, provides for their basic physical needs, and documents their homelessness to help them access benefits.
How You'll Help Fulfill the HSHM Mission
- Drives a company vehicle to locations not meant for human habitation and / or tent encampments with an Outreach Team member to provide mobile case management that identifies barriers and seeks to meet the physical, emotional, and spiritual needs of those experiencing unsheltered homelessness through a client-centered approach.
- Builds relationships with people who are experiencing unsheltered homelessness by talking with them, praying for them, and encouraging them to engage in services.
- Conduct screenings and assessments to aid client referrals and coordination with agency resources
- Provide services for people who are experiencing unsheltered homelessness at The Junction Community Center. Services include preparing clients to be document ready, connecting clients to emergency assistance (food, clothing, transportation, medical care, etc.), and connecting them to other services as appropriate.
- Coordinate and assist in the training, direction, and supervision of interns and outreach volunteers
- Partner and collaborate with other agencies and nonprofits to better serve and advocate for clients.
- Record client information, progress notes, case plan, and services rendered into the client services database. Record information necessary for outcome tracking.
- Document clients who are classified as homeless by HUD in the Homeless Management Information System (HMIS) for Coordinated entry, with all supporting documentation.
- Develop knowledge base of and partnerships with publicly supported programs and guidelines offered by local, state, and federal agencies.
- Work with Director of Community Outreach to inform the development, implementation and evaluation of programs and special projects.
- Represents Home Sweet Home Ministries in a positive light with all donors, recipients, clients, volunteers, and the general public.
