Home Therapy is seeking a Full-Time care coordinator to join our in-person office staff.
Position Overview :
- You will be responsible for responding to incoming referrals, intake, and monitoring a caseload of patients on the program.
- Additional tasks include insurance verification, answering and screening phone calls, communicating with therapists by phone and email, reporting to your supervisor, and attending weekly case management meetings.
