At Home Assist Health, our Direct Care Workers are instrumental in providing quality, independent lifestyles for people living with disabilities, and ensuring that they have the best care experience possible.
Under the direction of the Member Care Supervisor, this position provides assistance to meet the non-medical needs of members in their homes. Direct Care Workers assist members with personal care activities: routine house cleaning, laundry, ambulation, transfers, meal preparation, assistance with feeding, companionship, and assistance with learning objectives as described in the service plan. This position requires traveling between members homes. The employee must have reliable, insured transportation or the ability to utilize the transit system to ensure clients scheduled hours of care are covered. Working hours will vary and may require evening, weekend and holiday hours.
Job Functions
- Appropriately assists member with personal care activities, routine house cleaning, laundry, ambulation and transfers, meal preparation and feeding as indicated on Service Plan.
- Provides general supervision of the member and companionship in accordance with the members service plan as outlined by the assigned Member Care Supervisor.
- Coordinates shopping and accompany clients on errands, doctor/clinic appointments and assist with arranging transportation as needed and/or as approved.
- Accurately document services provided and complete data sheets/electronic visit verification in accordance with policy and service plan requirements on day that services are rendered.
- Immediately report any changes in clients condition or incidents to the assigned Member Care Supervisor or Supervisor on Duty.
- Assignments are fulfilled with reliability and punctuality utilizing personal conduct guidelines outlined in the Member/Caregiver Agreement.
- Maintain confidentiality regarding all personal client information.
- Maintain training/certifications as required.
- Maintain a professional appearance and adhere to dress code.
- Conform to ongoing background/fingerprint/attestation/affidavits as required.
- Performs other duties as assigned.
Qualifications
Education
- Minimum of an 8th grade education, High school education or GED preferred.
Knowledge, Skills and Abilities
- Requires the ability to read, write and speak effectively in English. If unable, must provide interpreter for interview, physical, and training classes.
- Good communication skills and the ability to follow directions, policies and procedures.
- Experience providing personal care and housekeeping, prefer three months to one year of experience as a Caregiver.
- Additional experience or training may be required for specific services.
- Reliable, insured transportation or the ability to utilize the transit system to ensure clients scheduled hours of care are covered
- Must be at least 18 years of age.
- Bilingual a plus.
Training/Certification/Licensure
- AHCCCS Direct Care Worker Training or current AZBN CNA/LNA.
- Current CPR and First Aid Certification.
Background Requirements
- Must have DPS Level 1 Fingerprint Clearance (or be able to obtain one within 90 days hire).
- Must pass Criminal Background if applying for DPS Level 1 Fingerprint Clearance Card.
- Must pass APS Registry Check at new hire and annually.
- Must sign notarized Criminal History Affidavit at new hire and every year.
Immunization Requirements
- Must have proof of TB Clearance.
Classification
This position is classified as Non-Exempt as outlined by the Fair Labor Standards Act (FLSA).
Working Conditions
- Must be able to lift 25 lbs.
- May require sitting or standing for long periods of time.
Direct Reports
Reports to: Member Care Supervisor, Assistant Program Manager, Program Manager, Program Director.
