What are the responsibilities and job description for the Assistant RN Clinical Manager position at Homebound Health Services, Inc.?
The Assistant Clinical Manager is responsible for directing the clinical activities of the agency; planning clinical services to patients and conferring with clinicians to assure services are provided at quality levels consistent with professional standards of practice. This position reports to the Clinical Manager and is qualified to act in the absence of the Clinical Manager.
Job Duties
- Ensure patient care is delivered according to established care guidelines, Plan of Care, physician orders, agency policies and procedures and meets insurance and payer requirements.
- Plans, directs and participates in the delivery of patient care services, including case assignment and visit scheduling activities.
- Establishes and implements priorities for services, when necessary.
- Recommends, implements and interprets policies and procedures relevant to the provision of patient care services.
- Serves as clinical resource to employees.
- Participates in the intake and referral process to receive complete clinical information and assure patients are accepted for care according to company admission criteria.
- Reviews clinical record documentation to assure compliance with state and federal regulations and company policies and procedures.
- Communicates effectively with Case Managers and employees to monitor the changing needs of the patient and the family.
- Participates in the after hours on-call process to ensure patient care policies and procedures are followed and staffing issues resolved.
- Serves as a liaison between the agency and the patient and family.
- Develops and maintains a good working relationship with referral sources including physicians, hospitals, nursing homes, community resources, insurance companies and payers.
- Establishes an environment that fosters the development of professional and paraprofessional employees.
- Monitors the skills and services of agency employees.
- Maintains on-going communication with agency employees.
- Communicates with the Clinical Manager with regard to the scheduling needs and concerns of patients and families.
- Portrays role modeling, teamwork, accountability and a positive attitude.
- Keeps Clinical Manager informed of all critical client care information.
- Performs other related duties and responsibilities as assigned by the Clinical Manager.
- Knowledge of Homecare Homebase or a comparable EMR and be computer/tech savvy.

Pay: $100,000.00 - $120,000.00 per year
