Job Summary
We are seeking a compassionate and dedicated Home Care Nurse to provide high-quality care to patients in their homes. The ideal candidate will have experience in home community care, particularly with patients requiring assistance due to conditions such as dementia. This role involves working closely with patients and their families to ensure a safe and comfortable living environment while promoting overall health and well-being.
Duties
- Conduct patient assessments, including vital signs monitoring and health evaluations.
- Administer medications and manage treatment plans as prescribed by healthcare providers.
- Provide personal care assistance, including bathing, grooming, and meal preparation.
- Observe and document patient conditions, reporting any changes to the healthcare team.
- Implement safety protocols in compliance with HIPAA regulations to protect patient privacy.
- Offer support for daily living activities in an assisted living environment.
- Perform first aid and CPR when necessary, ensuring patient safety at all times.
- Engage with patients in meaningful activities that promote mental and emotional well-being.
Qualifications
- Valid nursing license with experience in home community care preferred.
- Knowledge of dementia care practices is highly desirable.
- Proficient in vital signs assessment and medication administration techniques.
- Strong understanding of HIPAA regulations and commitment to patient confidentiality.
- Ability to perform first aid and CPR; certification is a plus.
- Excellent observational skills with the ability to communicate effectively with patients and families.
- Cooking skills are beneficial for meal preparation for patients requiring dietary assistance.
