The Scheduler serves as a liaison between the patient, agency, and clinicians and is responsible for coordinating and maintaining the scheduling for Agency patients and staff. The Scheduler will manage the flow of patient care, ensuring that quality service is provided and patient satisfaction met. Responsible for the scheduling and productivity of the Agency’s clinicians. Ensures appropriate communication with patients and clinicians to ensure timely starts of care and regular visits. Assists the Clinical Manager with other team projects are required.
Essential Job Duties AND Responsibilities
- Monitors internal workflow system to identify all patients completed by the Admissions Department.
- Ensures Agency patients receive timely care in accordance with their physician’s orders, services required, identified visit frequencies, and the Agency plan of care.
- Creates/maintains schedules and related data in the Agency’s electronic medical record.
- Ensures high patient satisfaction though clear communication and confirming demographic information, best times for visits, and best contact info for further scheduling.
- Assists clinicians by making sure they have all available information prior to the Start of Care.
- Review schedules to troubleshoot scheduling conflicts and issues. Reschedules missed visits as soon as possible.
- Maintains weekend and holiday rotation in accordance with Agency policies and Clinical Manager direction.
- Helps improve Agency profitability by only scheduling authorized visits and minimizing missed visits.
- Maintains staff and client statistics and reports.
- Communicates with the sales team regarding staff availability.
- Maintains strong collaboration with Admissions Department to troubleshoot obstacles preventing Starts of Care.
- Maintains clinician productivity in accordance with Agency standards.
