PROGRAM / LOCATION : Santa Clara County, Boccardo Reception Center
SUPERVISOR : Program Manager
JOB TYPE : Full Time, Non-Exempt

ROLE SUMMARY :
A Housing Retention Case Manager is responsible for providing comprehensive support services to help individuals and families participating in permanent supportive housing programs. Their intensive and individualized case management services are coupled with linkage to a network of community resources to assist clients in averting homelessness, maintaining housing, and increasing self-sufficiency. To this end, the Housing Retention Case Manager participates in our collective effort to end and prevent homelessness.

The Housing Retention Services Case Manager is passionate about serving others and sees themself in a helping profession long term. They are pro-active and responsive to the needs of our participants, meeting them "where they're at" and bridging the gap between the individual and our services. They manage their time well and demonstrate strong organizational abilities. With the support of program leadership, the Case Manager participates in our culture of learning and increases their knowledge of evidence-based practices over the course of their employment.
