Job Overview:
The Healthcare Liaison for multiple Senior Assisted Living Facilities serves as the primary point of contact between the facility, residents, families, and healthcare professionals. This individual will be vital in coordinating care for residents, ensuring seamless communication between healthcare providers, families, and facility staff, and promoting high-quality care and services for seniors. The Healthcare Liaison will also assist in community outreach, develop relationships with healthcare partners, and support the facility's mission to provide compassionate, personalized care.
Key Responsibilities:
- Resident Care Coordination:
- Work closely with healthcare providers, families, and facility staff to develop and implement personalized care plans for each resident.
- Ensure that resident health assessments are conducted and regularly updated in coordination with doctors, nurses, and other specialists.
- Act as a liaison between residents, family members, and healthcare professionals to address health concerns and coordinate treatment.
- Healthcare Provider Collaboration:
- Establish relationships with local healthcare providers, physicians, hospitals, specialists, and other healthcare entities.
- Assist in coordinating medical appointments, follow-up care, and transportation to ensure residents receive timely medical attention.
- Ensure that healthcare services meet regulatory requirements and facility standards.
- Family Support and Communication:
- Serve as the main point of contact for families regarding their loved one's healthcare needs.
- Communicate effectively with family members about health conditions, treatment plans, and any changes in a resident’s status.
- Organize family meetings as needed to discuss care plans and address concerns.
- Quality Assurance:
- Monitor care delivery to ensure compliance with healthcare standards, facility policies, and state/federal regulations.
- Participate in regular audits and assessments of resident care to identify areas for improvement.
- Report any concerns or deficiencies related to healthcare services to management for prompt resolution.
- Community Outreach and Networking:
- Represent the facility at healthcare-related community events, workshops, and meetings to promote services and build relationships.
- Develop referral relationships with local hospitals, clinics, and physicians to enhance service offerings and ensure a steady referral network.
- Promote the facility’s services to prospective residents and their families through outreach and educational materials.
- Resident Advocacy:
- Advocate for resident needs, ensuring they receive the highest level of care and attention.
- Assist residents and families in understanding healthcare options and resources available to them.
- Address and resolve any issues or concerns related to healthcare services or resident care in a timely and professional manner.
- Administrative Duties:
- Maintain accurate and up-to-date records of healthcare-related communications, care plans, and appointments.
- Prepare reports on resident health trends, satisfaction levels, and service outcomes for facility management.
- Assist in developing policies and procedures related to healthcare services within the facility.
Qualifications:
- Education & Experience:
- Experience in healthcare, senior care, or assisted living settings, with a strong understanding of senior care and services.
- Previous experience in liaison or case management roles, ideally in a senior healthcare environment.
- Able to travel to buildings that we have established in locations such as: Lapeer, Flint, Davison, Sagniaw, and possibly Bay City
- Skills & Abilities:
- Excellent communication and interpersonal skills to engage with residents, families, and healthcare providers.
- Strong organizational skills with the ability to manage multiple priorities and tasks.
- Knowledge of healthcare regulations, senior care services, and quality improvement processes.
- Ability to work independently and as part of a multidisciplinary team.
- Compassionate, patient, and professional demeanor when interacting with residents and families.
Physical Demands:
- Ability to sit, stand, or walk for extended periods.
