Job Type: Full-time
Pay: $60.00 - $68.00 per hour
Expected hours: 31 – 42 per week

Responsibilities:
- Creates an individualized, evidence-based plan of care
- Performs procedures appropriate type of services provided at the standard of care
- Orders and prescribes appropriate medications
- Conduct patient assessments and develop treatment plans
- Maintains patient confidentiality
- Document patient information accurately and timely in electronic medical records
To be successful in this role, you should have the following qualifications:
- Required PA or NP licensure in the state of Ohio
- Ability to work well independently evaluating and managing patients
- Ability to work in a fast-paced environment
