Job Title: Compassionate Caregiver – Make a Positive Impact Every Day!

Are you passionate about making a difference in the lives of others? We’re looking for dedicated, compassionate caregivers to join our team and help provide the highest level of care to individuals in need. As a caregiver, you’ll play a key role in supporting clients to live independently and comfortably in their own homes.
You’ll work closely with clients to follow personalized care plans, providing services such as companionship, meal preparation, light housekeeping, transportation, and assistance with personal care, including bathing and dressing.

Qualifications:
- Experience: At least 1,000 hours of experience with geriatric or chronically ill individuals is preferred. Exceptions may be made for CNAs, HHAs, or those focused on companionship and housekeeping.
- Certifications: CNA or HHA certification is a plus but not required.
- Health and Safety: Proof of current TB testing, ability to assist with client transfers, and compliance with infection control practices.
- Transportation: Reliable transportation, a valid driver’s license, and automobile insurance are necessary for client transport.
- Background Check: Must pass a criminal background check (both national and state-required).
- Communication: Strong communication skills are essential for interacting with clients, family members, and team members.
Key Responsibilities:
- Implement Care Plans: Follow personalized care plans to ensure clients receive the care and support they need.
- Provide Support: Assist clients with companionship, meal preparation, light housekeeping, transportation, and personal care (bathing, dressing, toileting, feeding).
- Advocate for Clients: Report any changes in client conditions to the Care Manager and advocate for their needs.
- Follow Safety Protocols: Adhere to safety procedures and infection control practices, and notify the office if additional supplies are needed.
- Accurate Documentation: Log hours, tasks, and client status accurately using the ClearCare Telephony system.
- Professionalism: Maintain a professional appearance and follow company policies and procedures.
Physical Requirements:
- Ability to assist with client transfers using proper techniques and equipment.
- Reliable transportation to clients’ homes.
