What are the responsibilities and job description for the Hope Studios Support Liaison position at Hope Community Resources, Inc.?
The Hope Studios Support Liaison provides services to Hope support recipients through their work in providing case management, assessment, planning, coordination, evaluation, and advocacy for options and services available in Hope Studios. The Hope Studios Support Liaison serves as a member of the multi-disciplinary team to develop goals and objectives as outlined in each recipient’s Plan of Care, Individualized Educational Plans, and/or other support plans, utilizing strategies in alignment with Hope’s organizational culture and service philosophy. This includes ensuring the services address health and safety needs of support recipients and provides positive learning, inclusion and engagement opportunities by utilizing knowledge of the programs that Hope provides through the Discovery Center. The Hope Studios Support Liaison maintains loyalty to the organization, its beliefs, values, mission, vision, and expectations and to the recipients of services.
Education and Experience
- Associate’s degree in Health Science, Social Science, Special Education or related field required; and,
- Two years of experience working in social services, one year of which involves responsibility for case management or service coordination required.
- Equivalent combination of related education and/or work experience.
Additional Requirements
- Possesses a valid Alaska driver’s license and has at least one year of driving experience (may include permit time). Obtains a driving record at time of employment and as requested by management.
- Able to safely operate Agency vehicles when needed. Possesses reliable transportation and vehicle insurance when transporting support recipients or conducting Agency business.
- Meets State and Agency requirements for passing a criminal background check upon hire and as required thereafter.
- Lifts 25 pounds from a squat and kneel position on the floor.
- Works at a desk/computer workstation for a minimum of six hours per day.
- Completes all required State, Agency, department, and position specific training as assigned.
- Obtains a tuberculin clearance at time of employment and annually thereafter.
