Position Title : Homeless Outreach Program (HOP) Manager
Program(s) and Location : Homeless Outreach Program (Sacramento, CA)
Schedule : Full-time, Exempt, 40 hours per week, Monday - Friday 8 : 00am-4 : 30pm (may require working some evenings and weekend schedules)
Pay rate : $2,860.11 semi-monthly ($68,642 annually)
About the Position :
The Homeless Outreach Program Manager is responsible for all aspects of Homeless Outreach and Training in accordance with Sacramento County outreach standards. The Program Manager serves as a liaison between the City of Sacramento, appropriate referring partners, and the Motel partners. Essentially, this position interfaces with partner organizations to assist with and coordinate temporary housing, encampment outreach and case management services for individuals who are homeless or chronically homeless with the goal of helping people to attain necessary services and / or permanent housing. The chief goal is to improve quality of life for the individual and assist to obtain and remain in stable or permanent housing.
Essential Duties & Responsibilities :
- Support with the development of program training (internal and external) for new hires, ongoing employee, and community partners
- Provides daily supervision of Outreach efforts
- Ensure the daily accuracy and timely completion of data collection and entry, reports, and client charts
- Ensure the review, evaluation and screening of relevant employment applications. Interview and check references on job applicants. Make hiring recommendations to the appropriate management team member.
- Assist with assigned organizational development activities, grant writing and other major projects.
- Requisition and purchase household, maintenance, office and program supplies as applicable
- Act as a designated Mandated Reporter for the observed or suspected abuse and / or neglect of children, disabled individuals, dependent adults and elder / senior citizens. Mandated Reporter also includes “Duty to Warn” which includes the responsibility to notify authorities and the intended victim(s) of physical harm.
- Assist in client concerns and complaints, intervene in, and / or oversee, crises and emergencies as they occur
- Transport clients as needed
- Ensure the protection and confidentiality of all Protected Health Information (PHI) by following all policies and procedures in the Agency HIPAA Manual
- Complete HMIS training and be proficient in all its aspects
Supervisory Responsibilities :
Directly supervises employees in the assigned program while reporting to the Program Manager. Carries out supervisory responsibilities by the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.
