- Patient Care and Assessment :
- Conduct initial patient assessments, including vital signs, medical history, and current health status.
- Administer medications, immunizations, and treatments following established protocols.
- Monitor patient progress, provide health education, and offer support and counseling as needed.
- Collaborate with healthcare providers to develop and implement individualized care plans.
- Clinic Support :
- Assist healthcare providers with examinations, procedures, and minor surgeries.
- Prepare and maintain medical equipment, ensuring their functionality and cleanliness.
- Maintain accurate and up-to-date patient records, documenting all interactions and interventions.
- Perform basic laboratory tests and specimen collection as required.
- Coordinate referrals to other healthcare professionals or community resources when necessary.
- Patient Advocacy and Education :
- Advocate for patients' rights, needs, and preferences, ensuring their voices are heard and respected.
- Educate patients and their families about health conditions, preventive care, and self-management techniques.
- Promote healthy lifestyles and disease prevention strategies within the community.
- Collaboration and Teamwork :
- Collaborate effectively with multidisciplinary teams, including physicians, nurses, and support staff.
- Participate in team meetings, case conferences, and quality improvement initiatives.
- Foster a positive and supportive work environment, promoting open communication and teamwork.OFF-SITE WORK : Although it is not typical, Off-Site Work could, at times, be required of this position. This off-site work will typically include, but is not limited to, performing nursing duties (i.e. Vital Signs) at Health Fairs or other HOPE Sponsored Events and attending off-site meetings, trainings or conferences.SKILLS / QUALIFICATIONS :
- Valid LPN license in Tennessee.
- Proven experience as an LPN in a healthcare setting, preferably in an FQHC or primary care clinic.
- Sound knowledge of nursing principles, practices, and procedures.
- Proficient in conducting patient assessments and providing direct patient care.
- Strong communication and interpersonal skills.
- Ability to work collaboratively as part of a team.
- Familiarity with electronic medical records (EMR) systems.
